基于模糊关联规则的大黄剂量与功效间关系研究

Study on the dose-efficacy relationship of Dahuang (Rhei Radix et Rhizoma) based on fuzzy association rules

    目的 基于模糊关联规则探讨大黄的量效关系,为其临床精准应用提供科学依据,并为中药量效关系研究建立新范式。
    方法 从中医古籍数据库筛选得到以大黄为君药且有明确剂量记载的2449首方剂。对功效术语进行规范化与合并处理,将历代剂量单位按所处朝代统一换算为公制单位(g),构建大黄量效方剂数据库。按患者年龄与给药途径将方剂分为6组,其中内服成人组(1828首)、内服小儿组(280首)、外用成人组(272首)纳入分析,其余3组因样本量不足予以剔除。按2∶6∶2的等频比例将剂量划分为小、中、大三个模糊区间,定义三角形隶属度函数对剂量数据进行模糊化处理,并采用模糊关联规则挖掘算法挖掘大黄的量效关系。
    结果 大黄用量随朝代更替呈下降趋势,汉代及唐宋时期多用较大剂量,明清以后逐渐接近现代临床用量。内服成人组中,小剂量主要关联消肿、通经、降气、解毒、行药力等功效;中剂量关联清热、泻下、疏风;大剂量关联破血、攻热、托脓。外用成人组中,小剂量主要关联消肿、止痛、生肌;中剂量关联清热燥湿、散结、消肿;大剂量关联托脓生肌、敛疮、固齿。内服儿童组中,小剂量以清热、解毒、泻下为主;中剂量以退黄、消疮、除肺热为核心;大剂量则偏向杀虫、涤痰、消积。
    结论 本研究运用模糊关联规则定量阐明了大黄内服与外用的量效关系,揭示了儿童较成人用量更低且功效侧重各有不同的特点。该方法能够有效处理中药剂量数据的连续性与模糊性,为量效关系定量研究提供了可复制、可推广的范式,有助于推动大黄的临床精准应用。

     

    Abstract:
    Objective To investigate the dose-efficacy relationship of Dahuang (Rhei Radix et Rhizoma) using fuzzy association rules, provide scientific evidence for its precise clinical application, and establish a novel paradigm for research on the dose-efficacy relationship of Chinese materia medica (CMM).
    Methods A total of 2 449 prescriptions with Dahuang (Rhei Radix et Rhizoma) as the sovereign drug and with explicit dosage records were screened from the Traditional Chinese Medicine Ancient Prescription Database. Efficacy terminology was standardized and consolidated, dosage units were uniformly converted into metric units (g) according to their respective dynasties, and a Dahuang (Rhei Radix et Rhizoma) dose-efficacy prescription database was constructed. The prescriptions were divided into six groups according to patient age and route of administration; the internal-use adults group (n = 1 828), the internal-use children group (n = 280), and the external-use adults group (n = 272) were included in the analysis, while the other three groups were excluded owing to insufficient sample sizes. Dosages were partitioned into small, medium, and large fuzzy intervals at an approximate equal-frequency ratio of 2 : 6 : 2, triangular membership functions were defined to fuzzify the dosage data, and a fuzzy association rule mining algorithm was applied to mine the dose-efficacy relationships.
    Results The dosage of Dahuang (Rhei Radix et Rhizoma) exhibited a decreasing trend across dynasties, with a relatively large dose being commonly used during the Han and Tang-Song periods, gradually approaching modern clinical dosages from the Ming-Qing period onward. In the internal-use adults group, small doses were mainly associated with reducing swelling, unblocking the channels, descending Qi, resolving toxins, and enhancing medicinal potency; medium doses were associated with clearing heat, purging, and dispersing wind; and large doses were associated with breaking blood, attacking heat, and expelling pus. In the external-use adults group, small doses were mainly associated with reducing swelling, relieving pain, and promoting tissue regeneration; medium doses were associated with clearing heat and drying dampness, dissipating masses, and reducing swelling; and large doses were associated with expelling pus and promoting tissue regeneration, healing sores, and strengthening teeth. In the internal-use children group, small doses were dominated by clearing heat, resolving toxins, and purging; medium doses centered on relieving jaundice, eliminating sores, and clearing lung heat; and large doses leaned toward killing parasites, clearing phlegm, and eliminating food accumulation.
    Conclusion Using fuzzy association rules, this study quantitatively elucidated the dose-efficacy relationships of Dahuang (Rhei Radix et Rhizoma) for internal and external use, revealing lower dosages and distinct efficacy emphases in children versus adults. The method effectively handles the continuity and fuzziness of CMM dosage data, providing a replicable and scalable paradigm for quantitative dose-efficacy research and advancing the precise clinical application of Dahuang (Rhei Radix et Rhizoma).
